Unfortunately, due to unforeseen circumstances, The Good Clinic is temporarily suspending operations. We will be unable to serve as your primary care provider for the immediate future.
During this time, we will work to coordinate transferring your care to another provider by providing you with a copy of your medical record or transferring your records to the provider of your choice.
To request a copy of your medical records:
1. Log into the patient portal. Navigate to My Health on the left panel. Choose Health Records from the top menu. Enter the date range or “All Time” option. Enter your email and click submit. You will receive an email with a link to securely download your records.
2. Call 844-383-8689, select option 5, and request a medical records release. You will need to complete a Release of Information form to complete the request. The form can be found here. Fax the form to 833-596-2334.
To find a primary care provider, use your insurance company’s Find A Provider function on their website to search for available providers near you.
